Our Mission

The mission of Pain Management of Oklahoma is to serve our patients with a patient centered approach to the management of their pain medication. Our goal is to ensure your healthcare needs are met while your pain level is diminished, either through a referral to outside services or through the proper management of your pain medications. We want you to be celebrated, not tolerated!

Appt Checklist

Here’s what to bring with you to every appointment:
insurance card,
in-state ID,
medications in original bottle,
medical records,
and personal/family history.

Medical Records

When preparing for your appointment, please bring with you any X-ray, MRI, or other reports detailing the reason for your pain. If you do not have any of these reports, we will refer you to a facility approved by your insurance for this testing to be completed.

Mandatory Test

Since, we are a pain management clinic, a urine specimen will be mandatory at all visits. Please make sure your bladder is full at time of arrival. If a urine specimen is not obtained, you will be unable to take prescriptions with you at the conclusion of your appointment.

Pain Contract

A pain contract will be expected from each patient on the very first visit. This contract will be the expectations of our physicians in relation to the continuation of care you will receive. Please call or email us with any questions about this contract.


We will work diligently to obtain a referral requested for you by our physicians. Some referrals will need to be made by your primary care physicians, so you may receive a call from their office with questions regarding the referral(s). Patients will also have to pass a background check and PMP review. If you haven’t heard from our office within 7-10 business days regarding your referral, please call our us.


All co-pays, deductibles or any money due is to be paid at the time of services. Charges not covered by your insurance or not paid on the date of service will be billed via mail. Please contact our office if you experience any issues regarding payment of your bill. Our billing office will try to work with you regarding a payment plan. Patients without insurance will pay directly for our services at time of service.

After Hours & Holidays

No medications will be filled after hours. Urgent Care facilities are available for such circumstances. Please call 911 in the event of a life-threatening emergency.

Our office is closed on the following national holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
